To Join South Florida Divers, Inc 

Please do the following:

1) Print and fill out the Member Application Form 
(Select "Auto portrait/landscape" or "auto rotate" on print dialog box.)
(Fill out a separate form each family member if you select a family membership.)

2) Make a copy of your c-card

3) Make a copy of your divers insurance

4) Please enclose a check for $70.00 made out to South Florida Divers Inc. for a single membership or

    $100.00 made out to South Florida Divers Inc. for a family membership.

     To pay with PayPal click on logo--->   PayPal When using PayPal you must specify
     Membership for the event.
     Failure will result in a delay in your membership.  For a video of instructions about

5) Bring along all four items to the monthly meeting (preferred) or mail it the following address:

Boca Raton, FL 33497-0082

As far as insurance, you are encouraged to purchase it from DAN (www.diversalertnetwork.org).

To renew your South Florida Divers, Inc membership

Please do the following:

(Select "Auto portrait/landscape" or "auto rotate" on print dialog box.)
(Fill out a separate form each family member if you select a family membership.)

2) Make a copy of your c-card

3) Please enclose a check for $70.00 made out to South Florida Divers Inc. for a single membership or

$100.00 made out to South Florida Divers Inc. for a family membership.  If the application is made after 

the February  meeting,  add $5 to the check amount.  $75 for single and $105 for family.
